One-Time Elective Form

These are the forms for students who want to take GLBE 451 (Research in Global Health) or another one-time elective such as FMME 299 or 499 or GLBE 499.  The student should fill it out and then submit it to UNC faculty member who will be grading the course.  (For most this may be the OGHE Director – Sylvia Becker-Dreps.  Then the signed form will be returned to the student to get additional signatures and/or submit it to the SOM registrar team so they can be officially registered for the course.

Preceptor or Supervisor Mid-Point and Final Evaluation Forms

Provide these to your on-site preceptor at the mid-point and prior to the end of the block. Preceptors can fill them out electronically and email or fill out on paper and scan them.  They should be sent to sbd@email.unc.edu or oghe@med.unc.edu
